Deductibles and Copays

Due to the recent changes in healthcare policies and practices, and the increase in High Deductible Health Plans, our practice will now be collecting copays at the time of service, even if you have not met your deductible. Please be prepared to make this payment at the time of your appointment. This payment can be made by cash, check, credit card and HAS/FSA accounts.

We will continue to submit claims to your insurance company. Once the claim is processed, any remaining balance will be billed to you. If you have any questions or concerns regarding this new policy, please contact the office at 716-635-0692.

Payments

Payments are expected at the time of service. We accept cash, check and all credit cards.

Form Requests

  • R&B Medical Group is happy to provide forms and copies of medical visit reports at your request.
  • We require a $10 fee per form for administrative costs.
  • Please allow a minimum of 5 business days advance notice for completion.

Cancellations/Missed Appointments

  • We require 24 hours advance notice for cancellations.
  • Due to an increased volume of missed appointments, there is a $40 fee for follow up appointments and a $50 fee for physicals, hospital follow ups, and preoperative consults missed or cancelled within the 24 hour time period.
